I understand Midlife Rx is a direct-pay practice. Insurance is NOT billed for visits, medications, lab, and prior authorizations are not completed.

I understand monthly membership is an ongoing subscription and is required for access to care.

I understand membership includes longer visits, direct communication with my provider, virtual care, and discounted pricing on lab and medications.

I understand prescriptions and medications are not included in the membership fee.

I understand treatment recommendations are individualized and not every patient is a candidate for hormone therapy or weight-loss medications.

I understand lab tests may be required before medications are prescribed or adjusted.

I understand all controlled substances require an in-person visit when required by Ohio and Federal Law.

I understand initial consultations and required baseline lab package totals $400 and are due before my appointment.
